Copies of the following documents must be attached with the application Form for a long term employment visa.

 

• Cover letter – details of child and reasons for visa application. This is to be signed by sponsor
•  FORM 17-FAMILY AND FOREIGN NATIONAL DEPENDANT SPONSORSHIP FORM
• Passport photo (If applying from overseas scanned color copy of application form with passport photo attached must be submitted)
• Passport bio-data page
• Birth Certificate – certified copy
• Copy of Sponsor’s LTEV/SPV(V)

After filling out the visa application form and attaching the required documents, submission of the form to the Immigration Division will require payment of an application fee.
